1. COMMUNICATE WITH THE CONCERNED COLLEGE REGISTRAR OF YOUR INTENTION TO PROCESS YOUR COMPLETION (FOR INCOMPLETE GRADE).
- The student pays PhP50.00 per removal and/or completion form through the CLSU Cashier.
2. PREPARATION AND SUBMISSION OF COMPLETION/REMOVAL FORM
- The student shall present the Official Receipt to the College Registrar for the issuance of Completion and/or Removal Form.
- The College Registrar shall supply the needed information in the Completion/Removal form and forward it to the faculty handling the subject.
- The Faculty shall indicate the grade in the Completion/Removal form, sign the form, and forward it to the Department Head.
- The Department Head shall validate and sign the form and then forward it to the College Dean.
- The College Dean shall review the Completion/Removal form.
- If the Dean finds the forms in order, he/she shall sign the form and endorse it to the College Registrar for submission to the Office of Admissions for encoding of grades.
Reminder:
Only the Completion/Removal Form endorsed by the College Dean and Directors of ISPEAR and NSTP shall be accepted by the Office of Admissions.
